MidState Soccer Club invites you to apply for our 3rd Annual MidState Classic Cup which will be held at the Decatur Soccer Complex on April 20-22, 2012.

Please make checks payable to the “Decatur Park District”.
Applications cannot be accepted without full payment.

ACCEPTANCE CRITERIA:
• U8-U14 Teams: first come, first served. Classic teams will be accepted in favor of premier teams.
• Club teams in good standing with their respective league and state organizations.

ACCEPTANCE:
• Accepted teams will be updated throughout the application process with final posting on the Website.
• Preliminary schedules and maps will be posted around April 8, 2012.
• No refunds can be made after your team has been accepted into the tournament.
• If not accepted, your team will automatically be placed on the waiting list.

FRIDAY, APRIL 20:
Check-in to present player cards – 6:00 p.m. – 9:00 p.m. at the Decatur Outdoor Soccer Complex, 1 Educational Park Drive, Decatur, IL 62526. Bracket games if needed.

SATURDAY, APRIL 21:
Check-in to present player cards – 7:00 a.m. – 11:00 a.m. at the Decatur Outdoor Soccer Complex (teams must check in one hour before game). Bracket games for all age groups.

SUNDAY, APRIL 22:
Bracket games for all age groups.

The Illinois Youth Soccer / US Youth Soccer Olympic Development Program is the elite player identification program in Illinois and the United States. The US Youth Soccer Olympic Development Program has identified more players participating on professional and national teams than any other program.

Volunteers are at the heart of every successful organization, and MidState Soccer Club is no exception. With this in mind, if you are a caring parent, alumni member or friend/family of a player or coach, you are welcome and encouraged to contribute your special efforts and talents to the club! Your support is greatly appreciated and will help ensure a winning tradition for our teams.

Assistance will be needed for special events, concessions, uniform ordering/distributing and more. Please call the MidState Soccer Club office at 429-3472 to volunteer.

Directions to Rotary Park

From North:

From U.S. Rte. 51 South, continue south through Decatur and over the bridge (Lake Decatur). Proceed south out of town. You’ll go under an

overpass and pass Imboden Road on your left. Turn lane into Rotary Park is ahead about 1/2 mile on your left (east).

From South:

From U.S. Rte. 51: as you approach the south edge of Decatur, you’ll pass Grove Rd. and a large sign for Red Tail Run Golf Club on your right (east).Rotary Park is just ahead on your right. Entrance is directly off of Rte. 51.
